How do I get a copy of my hospital chart or information from it?
All National Maternity Hospital maternity and baby charts are now electronic and are held on the Maternal & Newborn Content Management System (MN-CMS). The MN-CMS Project is the design and implementation of an electronic health record (EHR) for all women and babies in maternity services in Ireland. This record will allow all information to be shared with relevant providers of care as and when required. For further information, please click here. Gynaecology charts continue to be in paper format but will be made electronically available only in a few years.
Please note no information will be given over the phone. GPs must fax in requests on headed paper to: Hospital Fax Number: (01) 676 6623
Patient signed consent must be given with these requests.
